Cell Phones - Members that have cell phones should learn how to use SMS (not email) feature on their cell phones.  This will often get through when you cannot call.

ICS-211 INCIDENT-CHECK-IN-LIST, This form is to be used by those serving in the roll as "Staging Officer". All those reporting to the staging area are recorded on this form.

ICS-214 ACTIVITY LOG, This form is to be used by "ALL" for all assignments. This form is used to record activities at your assigned location as they happen.

ICS-213 GENERAL MESSAGE FORM, This form is to be used by "ALL" for sending and receiving formal messages. Because most emergency responders are not amateurs, they do not know how to forms that are used by Amateurs in the NTS. This is the form that all emergency responders will see and use.

ICS-309 COMMUNICATIONS LOG, This form is used by "TEAM LEADERS". This form is keep used to record communications with team members and to record assignments issued.

PERSONAL EQUIPMENT, This is check list for RACES Members to use to help them be prepared for a call out.

ICS-205 MICHIGAN CITY RACES Communications Plan. Members should print this form and have it available for call out. This form contains operational radio frequencies along with their assigned Channel ID. For example channel Alpha 1 is 441.950 (W9LY), Bravo 1 is 146.500 simplex.

This is the Michigan City Radio Amateur Civil Emergency Service (RACES) resource page. Required forms for use during activation and other information can be found on this page. Some forms are required to be used by all RACES members. Other forms are required based on the roll you may be called to fill. We suggest these forms be downloaded and that you have printed copies ready for use. Other resource documents may be posted on this page.
